Mr. Resident X, a 90 years old gentleman, has been living in a Private Nursing Home since the Year 2016. He is diagnosed with Diabetes Mellitus Type 2, Osteoporotic Fracture, and Pancreatic Insufficiency. His medications include Lantus insulin injection, Diamicron oral tablet, Multivitamin, and Creon. He is aware of himself and knows his family members, however, unaware of his limitations. He is still able to communicate and express his needs/wishes. He requires assistance for his activities of daily living. Please note also that he is doubly incontinent. In terms of mobility, He can stand up with assistance however unable to walk, making him at risk for falls.
