Task

Using the case information provided on Interact2, you will be required to investigate the relationships between your patient’s demographics, diet, dental condition, and medical history. These relationships will help you to identify and discuss the social determinants impacting your patient’s health outcomes and provide an analysis of their current health behaviours. You will then be required to further elaborate on how you plan to manage this patient by treatment planning, implementing preventative strategies and applying models of behaviour change.

Use the following criteria when structuring your case analysis:

Clinical presentation: Provide an overview of the patient’s presenting dental condition and link this to their periodontal/caries/oral cancer/NCTL risk status. Briefly discuss the patient’s medical history and any medical conditions present or at risk of.Nutrition and health implications: Analyse the diet history provided and identify any nutrients in excess or at risk according to the current Australian Dietary Guidelines (ADG). Discuss how areas of nutritional deficit and/or excess may be contributing to the patient’s oral health risks and any medical conditions they may have. Based on the risks identified and your patient’s lifestyle, detail 3 nutritional changes you would recommend for your patient. These recommendations must be supported by the ADG and other relevant evidence-based literature.Social determinants of health: Analyse 3 social determinants of health that are contributing to the patient’s risk status. Information included is well supported by evidence-based literature and is contextualised to the case. – Behaviour analysis Justifies the patient’s presumed stage of change, and correctly applies a behaviour change theory to rationalise the patient’s health behaviours with relevance to their motivations and barriersTreatment plan and appropriate preventative strategies: Provide an appointmentbased plan which follows treatment phasing and details how preventative strategies will be implemented and built upon throughout the course of care. Demonstrates how to sustain patient motivation in the clinical setting.Prognosis: Provides a prognosis for your patient’s oral health with consideration to their current health behaviours, diagnoses, planned treatment options, and your recommended behaviour change and preventative strategies.

You must demonstrate a culturally responsive approach to patient care throughout the entire assessment, with meaningful consideration given to First Nation’s patient experiences in oral health care which is relevant to the patient’s individual circumstances utilised in a clinical setting.
