Aim
This unit aims to provide learners with the knowledge required to deliver housing support services for health and wellbeing.

Task
You work for a Housing Support service, providing housing support to about 100 people across a geographically dispersed area. Some of your service users live in remote rural locations and some in busy towns. Around 60% of your service users are over 75, and you also work with people who have mental health challenges, a degree of learning disability or issues with addiction.
The local authority which provides your funding has told you that all housing support providers are to be reviewed as funding cuts and mergers are planned. It’s not all bad news though, because the local authority is keen to reward innovative practice and has promised to release funds for new ideas that service users have asked for where possible.
Your manager is asking you to put together a report to help prepare for this review process. This should be in a standard report format with a title page and contents, introduction and conclusion and you should include headings and sub headings as appropriate. You can base the report on your own organisation or on research carried out on other organisations.
Do not conduct any interviews with service users for this assignment.
Your report should be and should include the following:
1. An introduction explaining the role of Housing Support in promoting health and wellbeing and explaining the type of services you provide to your clients. This section should also refer to the regulatory standards, which will include the Housing (Scotland) Act 2010, the regulation process of the Care Commission, Codes of Practice for the sector and the Health and Social Care Standards.
2. Summarise the key job roles and responsibilities in your Housing Support organisation and evaluate whether these roles effectively deliver the outcomes of the organisation in line with the regulatory standards set out above, outlining any areas for improvement.
To complete this section, you should make comparisons with job roles and responsibilities in partner organisations and assess the effectiveness of any partnership arrangements you have established, identifying any gaps in provision which you plan to address.
3. Assess the key requirements for successful partnership working that will be required to respond to the current challenges and provide examples of the organisations that your housing support service will need to establish or strengthen links with to be successful. This will include statutory and voluntary sector partners.
4. Summarise how personal outcomes are set and reviewed using a tool such as Outcomes Star or Better Futures model and evaluate how successful the tool is in promoting both a person centred approach and effective risk management. Include two examples of how using information gathered from the personal outcomes tool has impacted on service provision in your evaluation.
5. Explain how you plan to involve service users in designing and delivering new services. You should refer to two examples of good practice where an organisation has successfully involved service users and assess how successful these attempts have been, indicating how they can be used to design your own service user involvement. You should specify how success will be evaluated in your project.
