In this assignment, students will use the case of Omarina Cabrera from the videos.
Using class information, students will conduct a differential diagnosis and create a conceptualization and treatment plan based on the case study.

Important Notes:
You can embellish and make inferences in the vignette as long as additions/adjustments do not change the basic facts of the vignette.
Focus on Omarina at the age she is in that particular video. You’ll also want to focus on presenting problems discussed in that particular video. Focusing on the final video, you will have more context and background information about the stressors she has been going through over a span of several years.

Writing, page length, APA, etc.
The paper should be 5 to 7 pages in length, including a title page.
● Per general APA guidelines, this assignment should be typed and double-spaced, with 1″ margins on all sides, and 12 pt. Times New Roman font.
● Professional writing is expected of all students including spelling, grammar, syntax, etc.

Differential Diagnosis: 20 pts in total
● Students will utilize resources to determine the diagnosis based on the tools that they find including DSM V, Diagnosis Tree, research articles, etc.
● Students will discuss how they came to the conclusion and how it is different from other conditions that may have similar symptoms. (See Final Determination)
● Students are expected to choose AT LEAST TWO OTHER DIAGNOSES to consider, other than the chosen diagnosis.
○ Discuss symptoms/ presenting problems (3 pts)
○ Discuss consideration 1 (other diagnosis) – How the client’s symptoms met the criteria, but why not the diagnosis (4 pts)
○ Discuss consideration 2 (other diagnosis) – How the clients symptoms met the criteria but why not the diagnosis (4 pts)
○ Final Determination (chosen diagnosis) – Include the complete diagnosis with the ICD 10 code) (4 pts)
○ Professional writing – spelling, grammar, syntax, flow of thought, etc. (3 pts)

There is no page length requirement for the Differential Diagnosis portion of this assignment.

Case Conceptualization: 32 points in total
Based on information learned in this course, students will create a conceptualization for the adolescent in the case study including the following:
● Presenting problem(s) (4 pts)
● Contributing factors (4 pts)
● Developmental Factors (12 pts)
○ Physical
○ Social
○ Emotional
○ Cognitive
● Relevant Environmental and Personal experiences (4 pts)
● Conceptualization (explanation of what you think is happening for this client through your theoretical lens (4 pts)
● Professional writing – spelling, grammar, syntax, flow of thought, etc. (2 pts)
● 2 to 3 pages in length (2 pts)

The Conceptualization portion of this assignment (i.e., discussion of developmental factors, relevant environmental and personal experiences, presenting problems, contributing factors, and explanation through your theoretical lens) should be 2 to 3 pages in length (not including title page if you choose to include one).

Treatment Plan: 23 points in total
Based on information learned in this course, students will create a treatment plan for the adolescent in the case study including the following:
● At least two appropriate goals (4 pts)
● At least two objectives for each goal (8 pts)
● At least one intervention/technique for each objective (8 pts)
● Professional writing – spelling, grammar, syntax, flow of thought, etc. (3 pts)
